What is the sense of agency?

It is the fundamental, background feeling that you are the author of your own actions and thoughts. When you reach for a cup, you just know you moved your arm. In schizophrenia, this subtle neurological mechanism can break down.

How the connection is lost

According to the comparator model of motor control, the brain constantly predicts its own actions. Here is what happens when that system fails.

The brain sends a motor command to move or think, and simultaneously sends an efference copy (a prediction) to the sensory cortex.

Result: "I did that."

Due to altered connectivity, the "heads up" signal (prediction) is weak, delayed, or entirely missing.

The actual movement or thought occurs, but without a prediction it feels completely unexpected.

Result: "Someone else did that."

The brain rationalizes the unpredicted event as an external force, such as thought insertion or delusions of control.
